If you have a change of name or address, please let us know. You will need to bring a form of documented proof to reception so that we can verify the change. If your new address is no longer in our catchment area we will advise you register at a GP closer to your new address. You will then be removed from our system within 30 days.
It is your responsibility to let any other community services or hospital know about these changes.
